Below are a couple of more things for you to do. The following waivers (they are jot forms) need to be filled out:
• Photo waiver – only needed if you do NOT want your child photographed (see waiver for more details)
If your child has participated in other CISF programs in 2022, you do not need to fill new forms out. We have them in our files. Thank you!
